THR Journal Article Review - Avoiding Functional Decline and Minimizing the Effects of Frailty in Hospitalized Older Adults
This scholarly article review is designed to change the nurse’s skills and care strategy by improving their ability to identify frailty risk early (using practical screening approaches) and to use the Frailty Care Model to plan and implement consistent, targeted interventions for common modifiable drivers of frailty progression, especially mobility/physical activity, nutrition, and depression/social isolation.
