THR Social Isolation and Loneliness in Older Adults

Target Audience

This online interprofessional educational enduring material is designed for physicians, nurses, physician assistants, pastoral care, behavioral health and other members of the healthcare team. Its purpose is to address the need for identifying social isolation and the impact it has on patient health outcomes.

Learning Objectives

At the end of this online enduring material, participants should be able to:

  • Identify patients with social isolation and various resources including screening tools and referral tracking in the social isolation and list outcomes for seniors (SILOS)
  • Increase awareness across the healthcare system in the understanding of the health-related impacts of social isolation
  • Increase the ability to connect seniors with interventions to improve or reduce social isolation
